We had an outstanding week in Grand Rounds, led off by a visiting lecturer and UC EM Class of 2013, Dr. Renee Salas. Dr. Salas is currently Assistant Professor at Harvard Medical school and a Yerby Fellow at the Center for Climate, Health, and Global Environment. Her talk was entitled “Climate Crisis and Clinical Practice - Implications for Emergency Medicine.” Next, Dr. Baez presented a fascinating Ultrasound Grand Rounds with Ultrasound M&M. Drs. Frederick and Walsh led us through their new clinical pathway for Non-Pregnant Vaginal Bleeding in their QI/KT. Dr. Klaszky gave his R4 Clinical Capstone lecture on how to make effective presentations.

This week featured an all-start lineup of resident lectures. Dr. Golden started us off with an amazing M&M, followed by Dr. Mullen enlightening us on eating disorders and the management in the ED. Dr. Golden kicked grand rounds off this week with our monthly morbidity and mortality conference, followed by Dr. Lane’s deep dive on massive pulmonary embolism in cardiac arrest and how to approach this. Dr. Owens then gave us a detailed review on a probably-more-common-than-you-think parasite that has started popping up in the US, and our excellent ED pharmacists gave us a crash course on common antidotes to various poisonings, overdoses, and bites that we frequently see in the ED. Finally, Dr. Ham finished up with a review of the importance of chronic hypertension in our ED population and how we can make a difference especially those with limited access to care.

Dr. Banning led us through robust discussion about concurrent pathology presentations, biases and more. Air Care Grand Rounds followed, in which we reviewed aircraft operations and logistics with the Air Care team and went through a simulation case that highlighted the importance of stress inoculation.

This weeks grand rounds started off with a Morbidity and Mortality presentation by Dr. Baez including topics such as STEMI in LVH, pharmocologic cardioversion, and septic arthritis of the finger. This was followed by Dr. Freiermuth who gave us some pearls on the approach and management of sickle cell patients in the ED. Dr. Shaw then discussed some disturbing new health policy issues. Dr. Essell, a Heme/Onc attending at Jewish Hospital then walked us though GvHD, Acute Leukemia emergencies, and a fascinating new treatment option for blood cancers CAR-T. The day continued with a review of burn management by Dr. Spigner. Dr. Walsh then finished the day with an overview of carboxyhemoglobinemia and methemoglobinemia.

This week started with our monthly Morbidity and Mortality conference where we discussed posterior MIs, tough dissections and more tough cases. We then heard a debate on the use of D-Dimer in the diagnosis of aortic dissection. Finally, we were led through a simulation of a sick GI bleed requiring Minnesota tube placement, and we discussed optimal management of these challenging patients.

This week Dr. Lagasse gave us a great M&M covering intra-ocular foreign bodies, empyema management in the ED and septic cardiomyopathy. Dr. Iparraguirre lead small groups in EKG case studies in heart blocks, followed by Drs Nagle and LaFollette squaring off in a CPC of an undifferentiated patient with aphasia. Drs Goel and McKee wrapped up this week with a clinical soapbox on patient turnover and a case follow up of lytics in massive PE.

Dr. McKean kicked off this week with another great M&M where he taught us about the utility of stress testing to predict coronary artery disease, otomastoiditis, and much more. Dr. Brenkert joined us for an hour on pediatric musculoskeletal ultrasound and then Dr. Mudd reviewed transfusion strategies in upper GI bleeding. Dr. Ventura taught us about CSF analysis and Drs. Stettler and Whitford rounded out the day with a CPC about acid-base disturbances.

Dr. Grosso kicked off Grand Rounds this week with March M&M by diving deep into some core content, including BB and CCA overdoses, influenza, massive transfusion, post-intubation hypotension, and neurological catastrophes causing cardiac arrest. Dr. O'Brien broke down coagulopathy of liver disease and DIC for us while Dr. Golden taught us about febrile seizures. Drs. McKee and Colmer talked through the evidence behind their CPQE pathway on vent management in obstructive lung disease. Drs. Liebman and Powell went head to head in a CPC case about sternal osteomyelitis to round out another excellent week of learning.

Dr. Soria and Dr. Riddle gave us their R2 and R4 case follow-ups, respectively.
