What a great Wednesday at UCEM Grand Rounds! To start off the morning, we had the absolute honor to host NFL Players Association Medical Director Dr. Thom Mayer who guided us through a lesson on leadership via examples in the NFL. We then transitioned to our quarterly EMS grand rounds where we discussed updated Southwest Ohio EMS protocols with EMS fellow Dr. Weekley. Following this, we brought out our competitive sides by participating in a game of sports medicine jeopardy led by Dr. Betz, one of our sports medicine trained faculty members. Dr. Fabiano then gave an insightful capstone lecture on reflecting on what you want to do with your life, particularly outside of our careers. We then transitioned back to the clinical space for Dr. Snyder’s informative lecture on prosthetic valve emergencies. Finally, we wrapped up the day with a quality improvement and knowledge translation lecture about spontaneous intracerebral hemorrhage.

We start out this week with our Morbidity and Mortality conference with Dr. Stark where she dives into a variety of cases included dog bite related infections, critical care boarding, malaria, septic bursitis, lunate dislocations, and regional wall motion abnormalities. Next, Drs. Arnold and Wilson discuss workup and management of patients with acute liver failure in the emergency department. Dr. Moulds then followed where she discussed ventricular tachycardias including Torsades de Pointes during her Taming the SRU presentation. Finally, we wrapped up the day with Dr. Kein’s R4 Case Follow Up where she discussed a patient who presented three patient cases who deserve your conservativism.

What a fabulous and jam-packed Grand Rounds it was! Drs. Knight, Roche, and Freiermuth kicked us off with fantastic attending case follow-ups with pearls for physicians of all levels. Drs. Frankenfeld and Hill duked it out in a fascinating case of iron toxicity. Dr. Thompson expertly led us through UC’s new model for patient safety. Dr. Freiermuth brought us back to the basics of refining our history taking skills with tips on approaching sensitive topics. Dr. Pulvino walked us through a very difficult presentation of simultaneous stroke and STEMI in her R3 Taming the SRU. From surgical airways to the undifferentiated shock patient, this week’s Grand Rounds was packed full of clinical pearls. Dr. Carleton started with a discussion of a tachycardia-inducing failed airway requiring cricothyrotomy. Drs. Jensen and Makinen presented a very detailed review of the literature and their proposed algorithm on infective endocarditis. Dr. Harty reviews a fascinating case of cecal volvulus that was identified early with the aid of a RUSH exam, while Dr. Liebman walked us through an approach to the patient with inhalation injuries. Finally, Dr. Roblee led an excellent review of SBP. The discussion was full of information you might use on your next shift!

Welcome to another grand rounds recap! This week Dr. Isaac Shaw started us out with the monthly Morbidity and Mortality. Dr. Stolz then dove into some ultrasound QA, covering topics such as knee arthrocentesis and early pregnancy ultrasound. Dr. Murphy followed this up by discussing the science of motivation and how we can use this in the Emergency Department setting. This was followed up with Drs. Modi and Kircher who went head to head in this months CPC on endocarditis. Dr. Irankunda finished up the day with an excellent talk on the retrograde urethrogram. See you next week!

When a patient with a history of recent IVDU presents with a complaint of fever, the mind of the provider should immediately focus on the numerous possible infectious complications that can arise.  Infectious endocarditis can lead to septic emboli spread to any organ system.  Pneumonia can result from aspiration or septic embolization. Cellulitis/abscess can obviously result from local injection.  But what about when a source of fever is not readily identifiable? When cultures are negative and the patient’s symptoms have resolved, what could have been the cause of their febrile illness?

This week Drs. Jarrell and Nagel kicked off Grand Rounds with their evidence based review of the hypertension management in the ED. Drs. Gorder, Lagasse, O'Brien, and Polsinelli discussed the difficult management of the patient in both cardiogenic and septic shock, reviewed heart murmurs, and endocarditis. Dr. Bryant made us squirm with her Global Health quick hit case review of parasitology, Dr. Lane led a great group discussion about the utility of procalcitonin in adults, and Drs. Habib and Roche ended Grand Rounds with a great CPC case!
