We had a great Grand Rounds with nine lectures!

Dr. Fermann gave us operations updates for the CEC. Dr. Dave Thompson discussed quality improvement, root cause analysis, and the Pareto Principle. Dr. Tillotson walked us through the types of hypertension seen in pregnancy and how to manage these patients. Dr. Walsh found a sandwich in a patient’s airway. Dr. Jarrell discussed the exciting specialty called Social Emergency Medicine. Dr. Koehler showed us how to use a Minnesota tube for massive UGIB. Dr. Milligan discussed complex cardiac devices including IABP, Impella, and LVAD. Dr. Hassani reviewed post partum headaches and evidence on neuroprotective intubation. And lastly, Dr. Gleimer made us exercise our System 1 and System 2 thinking.

Dr. Golden kicked grand rounds off this week with our monthly morbidity and mortality conference, followed by Dr. Lane’s deep dive on massive pulmonary embolism in cardiac arrest and how to approach this. Dr. Owens then gave us a detailed review on a probably-more-common-than-you-think parasite that has started popping up in the US, and our excellent ED pharmacists gave us a crash course on common antidotes to various poisonings, overdoses, and bites that we frequently see in the ED. Finally, Dr. Ham finished up with a review of the importance of chronic hypertension in our ED population and how we can make a difference especially those with limited access to care.

We then split up into groups and did the quarterly sim focusing on informed consent led by Drs. LaFollette and Lang. This was followed by some challenging oral boards cases chosen by Drs. The final Grand Rounds of 2017 opened with Morbidity and Mortality led by Dr. Kari Gorder. Drs. Liebman, Banning, and Owens then shared exciting Global Health cases from their travels to Tanzania and Guatemala. Dr. Grace Lagasse gave her soapbox lecture on firearm violence and the Emergency Department. Finally, our dentistry colleagues Drs. McMahon and Dagher refreshed our techniques and skills in the management of dental trauma.

This week Drs. Jarrell and Nagel kicked off Grand Rounds with their evidence based review of the hypertension management in the ED. Drs. Gorder, Lagasse, O'Brien, and Polsinelli discussed the difficult management of the patient in both cardiogenic and septic shock, reviewed heart murmurs, and endocarditis. Dr. Bryant made us squirm with her Global Health quick hit case review of parasitology, Dr. Lane led a great group discussion about the utility of procalcitonin in adults, and Drs. Habib and Roche ended Grand Rounds with a great CPC case!

Grand Rounds kicked off this week with Dr. Axelson's final M&M of the year where we learned about hypertensive emergencies, 2nd & 3rd trimester vaginal bleeding, the care of the sick asthmatic, which bronchiolitics can go home and how exactly to treat the many forms of UTIs. Drs. Kircher and Murphy-Crews continued the learning with a case follow-up about intubating patients with airway stents and pediatric head injury, respectively. Our joint EM-Peds lecture rounded out the day with visual diagnoses in peds.
