We had another exciting week of Grand Rounds! We covered topics including our new hypoglycemia protocol with Drs. Beyde and Wolski to traumatic cardiac tamponade with Dr. Glenn to syncope roles with Dr. Kotei. We enjoyed a visiting lecture from our Ohio ACEP team talking about the role of ACEP in residency and beyond. We finished strong with a lecture from our program director Dr. McDonough talking about her take on wellness in residency!

One case ended up being a purulent pericardial effusion while the other was an LV freewall rupture. Dr. Sabedra Expanded our knowledge on how to better care for pregnant and breast feeding patients and how to be a support co-worker to our pregnant and breast feeding colleges. Lastly, Dr. Frankenfeld walked us through some of the data around when patients leave against medical advice, how to try to avoid this, and how to appropriately counsel our patients.

Read MoreFirst, pericardiocentesis should be considered a temporizing procedure. In the setting of trauma, you are hoping that the pericardiocentesis will clear a small amount of blood from the pericardial space and remove any tamponade the might be present. It is likely, however, because of the mechanism of injury, that blood will again rapidly accumulate leading to recurrent tamponade physiology. Ultimately (but not on Air Care — DON’T do a clamshell), these patients will need a pericardial window, exploration, and repair of whatever injury is causing the accumulation of blood.
