We started off this week with femoral nerve block case review with Dr. Lori Stolz, followed by our first ever morbidity and mortality conference focused on community practice given by Dr. Hughes. Next, Drs. Boyer and Benoit faced off in the clinical pathologic case lecture. Dr. Moulds covered high yield ophthalmology visual diagnosis, and Dr. Baxter discussed time sensitive vascular pathology. We finished off the day with Drs. Richards and Demel who discussed the operational aspects of stroke care.

Baez and Continenza faced off for the most recent installment of our Great Debate series as they discussed chemical vs electrical cardioversion for atrial fibrillation. Finally, our colleagues from Cincinnati Children’s presented learning pearls about causes of and interventions for hypoxia in pediatrics emergency medicine.

In this week’s Grand Rounds we discussed spinal fractures and imaging of knees and hips with our R1s, Drs. Kimmel and Gressick. Dr. Hassani from the R2 class took on Dr. LaFollette with a case of thyrotoxicosis presenting as a-fib with RVR in his CPC, and Dr. Koehler from the R3 Class taught us about heroin/naloxone-induced pulmonary edema. Dr. Golden from the R4 class discussed Fournier’s Gangrene complicated by sepsis-induced cardiomyopathy, and finally our trauma surgery colleague Dr. Pritts discussed some hot topics in trauma.

In this month’s Journal Club Recap we take a look at some recently published literature about common heart related complaints in the ED. First, we look at the now nearly ubiquitously used HEART pathway. In a US population, do the benefits of decreased health care utilization sustain themselves to a year out of an index visit? Then we turn our attention to atrial fibrillation with RVR. Does the utility infielder of ED medications, Magnesium, actually help with more rapid rate control? And, should the results of a consensus panel sway us to treat A fib with RVR as an outpatient?

This weeks Grand Rounds was started off with our guest lecturer, Dr. Esther Choo, who gave a compelling talk regarding gender disparities in medicine and the effects of gender bias in our profession and every day practice.  This was followed by Dr. Owens and Ventura giving their evidence based approach to the management of A-Fib, as well as previewing their upcoming algorithm for Tamingthesru.com.  Our pediatric EM colleague Dr. Valentino then walked us through current recommendations on the approach to the febrile infant.  In the last hour, Dr. Iparraguirre gave us a lecture on the approach to rashes in the ED.  The conference then concluded with an R4 clinical soap box lecture by Dr. Dang on the history of cannabis and medical marijuana, as well as the research behind its current use.

This week, we started things off with a great Quarterly Sim led by our faculty. The oral boards cases, led by Drs Stettler and Roche, involved an acute presentation of holiday heart, a post-partum patient with flash pulmonary edema, and a very questionable spider bite. The simulation, led by Drs Fernandez, Hill and Stolz, focused on two patients that were in shock: one due to a ruptured ectopic pregnancy and one due to a pericardial tamponade. We then moved on to the pediatric side of things, where Dr. Gleimer discussed neonatal rashes, and we took a look at pediatric syncope with Dr. Fananapazir.

We learned about injury management in resource-limited settings during Global Health Grand Rounds with Drs. Roche, Lagasse, Teuber. Dr. Soria and Dr. Riddle gave us their R2 and R4 case follow-ups, respectively.
