Another week, another great Grand Rounds line-up at UCEM. We start off with our monthly Mortality and Morbidity Conference led by Chief Resident Dr. Finney. This session covers cases from the month of April- including the treatment of a canine in the SRU. One of our SRU leaders, Dr. Chhabria, shares the changing landscape of cardiac arrest management using ECMO. Next up, Dr. Thompson helps us reflect on the self-talk that we engage in on a daily basis and offers advice on how to improve our mindset- especially when we are on-shift. We learn from Dr. Kotei how to better ED proceduralists when encountering patients with ascites and/or pleural effusions. We end with Dr. Hajdu challenging Dr. Baez to solve a puzzling case of progressive vision loss in a otherwise healthy young patient.

It was another exciting week here in Cincinnati as we kicked off grand rounds with a session on performance improvement during our quarterly leadership curriculum with Dr. McDonough and discussed alternatives to hospital admission and available outpatient resources in the era of high boarding with our Chair, Dr. Pancioli. We finished our day with an excellent discussion on prenatal mental health care with Dr. Wolochatiuk in her R4 Case Follow Up and on uncommon causes of disorientation in Dr. Zalesky’s Taming the SRU lecture.

What a fabulous and jam-packed Grand Rounds it was! Drs. Knight, Roche, and Freiermuth kicked us off with fantastic attending case follow-ups with pearls for physicians of all levels. Drs. Frankenfeld and Hill duked it out in a fascinating case of iron toxicity. Dr. Thompson expertly led us through UC’s new model for patient safety. Dr. Freiermuth brought us back to the basics of refining our history taking skills with tips on approaching sensitive topics. Dr. Pulvino walked us through a very difficult presentation of simultaneous stroke and STEMI in her R3 Taming the SRU. Dr. Fermann gave us operations updates for the CEC. Dr. Dave Thompson discussed quality improvement, root cause analysis, and the Pareto Principle. Dr. Tillotson walked us through the types of hypertension seen in pregnancy and how to manage these patients. Dr. Walsh found a sandwich in a patient’s airway. Dr. Jarrell discussed the exciting specialty called Social Emergency Medicine. Dr. Koehler showed us how to use a Minnesota tube for massive UGIB. Dr. Milligan discussed complex cardiac devices including IABP, Impella, and LVAD. Dr. Hassani reviewed post partum headaches and evidence on neuroprotective intubation. And lastly, Dr. Gleimer made us exercise our System 1 and System 2 thinking.

Dr. Dave Thompson then led us through the latest installment of the quality improvement series with insights for how to lead QI initiatives. Dr. Crawford reviewed tick-borne illnesses in her R1 clinical knowledge presentation, and Dr. Modi discussed highlights of management and prognostication of refractory v-fib arrest. Catheter Associated Urinary Tract Infections and Foley Alternatives

Catheter associated urinary tract infection (“CAUTI”) is one of the most common nosocomial infections in hospitalized patients. The use of external urinary catheters, the male external catheter, colloquially known as the “condom catheter,” and the wicking catheter, most often used in females, may help prevent the occurrence of CAUTI. In the Emergency Department, we are poised in a position where what we do (or don't do, such as insert a foley) has the potential to reduce iatrogenic harm throughout the hospital.
