It was another exciting week here in Cincinnati as we kicked off grand rounds with a session on performance improvement during our quarterly leadership curriculum with Dr. McDonough and discussed alternatives to hospital admission and available outpatient resources in the era of high boarding with our Chair, Dr. Pancioli. We finished our day with an excellent discussion on prenatal mental health care with Dr. Wolochatiuk in her R4 Case Follow Up and on uncommon causes of disorientation in Dr. Zalesky’s Taming the SRU lecture.

What a fabulous and jam-packed Grand Rounds it was! Drs. Knight, Roche, and Freiermuth kicked us off with fantastic attending case follow-ups with pearls for physicians of all levels. Drs. Frankenfeld and Hill duked it out in a fascinating case of iron toxicity. Dr. Thompson expertly led us through UC’s new model for patient safety. Dr. Freiermuth brought us back to the basics of refining our history taking skills with tips on approaching sensitive topics. Dr. Pulvino walked us through a very difficult presentation of simultaneous stroke and STEMI in her R3 Taming the SRU. Lastly, Dr. Mand capped off the day with her R4 capstone detailing the lessons she has learned throughout her residency training. Tune in now for a great read!

Check out this week’s recap of Grand Rounds! Dr. Tim Murphy took us through some fascinating cases with Morbidity and Mortality Conference. Next, we got to dive deep into toxicology. Dr. Kelli Jarrell led us through a case she had of a TCA overdose, Dr. Shawn Hassani taught us about Beta Blocker and Calcium Channel Blocker overdose, and Dr. Woods Curry took us through a oral boards session during Quarterly Sim reviewing Aspirin toxicity. Quarterly Simulation also had an oral boards case discussing inferior STEMI complicated by complete heart block, as well as an awesome simulation teaching the fundamentals of teamwork and closed loop communication by having a lucky R3 run a code with a blindfold on. We will all have the experience of taking care of a patient in the emergency department who is acting…different than they normally do.  Sometimes, the change can be subtle, maybe a family member will be the first to notice and bring the patient to be evaluated.  Sometimes the patient makes the change abundantly clear.  In either case it is essential to identify the underlying cause and treat any emergent conditions precipitating this dysfunction of the brain.  This month we hear from Dr. Erin McDonough on her approach to the patient with altered mental status (AMS).

#lessonslearned is a case series submitted by former senior residents describing B-pod cases that taught them the art of medicine

Case 1

A male in his 30s with a past medical history significant for hypertension presented via EMS with pain all over, anxiety and shortness of breath. The EMTs reported that he had smoked marijuana about 30 minutes before his presentation from his normal supply. At the scene he was noted to be very agitated, diaphoretic and vocal about his pain. He was yelling that he was hurting all over, he was having trouble breathing, and that he wanted to be sedated. He states that he has never had problems like this.
