Another week, another great Grand Rounds line-up at UCEM. We start off with our monthly Mortality and Morbidity Conference led by Chief Resident Dr. Finney. This session covers cases from the month of April- including the treatment of a canine in the SRU. One of our SRU leaders, Dr. Chhabria, shares the changing landscape of cardiac arrest management using ECMO. Next up, Dr. Thompson helps us reflect on the self-talk that we engage in on a daily basis and offers advice on how to improve our mindset- especially when we are on-shift. We learn from Dr. Kotei how to better ED proceduralists when encountering patients with ascites and/or pleural effusions. We end with Dr. Hajdu challenging Dr. Baez to solve a puzzling case of progressive vision loss in a otherwise healthy young patient.

Another excellent week for UCEM Grand Rounds! We started the day with an introduction to universal HCV screening in the ED from our pharmacy colleagues. We then transitioned to a great discussion on concussions sports medicine faculty Dr. Gawron. Next, Dr. Guillaume led us through diagnostics and therapeutics for nonconvulsive status epilepticus. Then, Dr. Shaw presented two cases of PRES and the importance of recognizing this diagnosis. Finally, R4s led the way through a simulation on adrenal insufficiency as well as structured oral boards case practice.

Learn about the art of skillfully resuscitating a patient with an LVAD, as well as the tips/tricks for effectively communicating with patients, from our senior residents. This week’s Grand Rounds was kicked off with our inaugural “The Art of EM” lecture that included a panel of our esteemed non-UC trained faculty members Drs. Lang, Minges, D.Thompson, Adan, and Stolz. Dr. Gillespie then expertly led us through hand infections in her R1 Clinical Knowledge lecture. Drs. Comiskey & Crawford took us on a deep dive of the literature surrounding the evaluation and management of DVTs. Lastly, our PEM colleagues walked us through a great video-simulation case series on critical pediatric cardiac pathology!

This week we had another great M&M where Dr. Klaszky covered exciting topics such as refractory V fib, hypothermia and cerebral malaria. We learned about misnomers, mimics and lies about TB from Dr. Owens. Dr. Zalesky skillfully walked us through a simplified approach to reading brain MRIs. Lastly, Dr. Leech and Dr. Goel faced off in a CPC involving a patient with ALS.

Grand rounds this week started off with a review of the EMTALA law with Dr. Hinckley, as well as some case discussion.  This was followed by a presentation from our clinical pharmacists, PharmD's Nicole Harger and Madeline Stephens, on anti-epileptic medications and their uses, as well as an update on our medication supplies in the ED.  Drs. Murphy-Crews and Scanlon then presented their evidenced based algorithm for the management of STEMI, followed by our combined peds EM lecture with Dr. Kevin Overmann on the evaluation of the pale child and pediatric anemia.  This was followed by Dr. Ludmer's R4 case follow up/best of residency cases.  The day finished up with Dr. Sim Mand presenting her clinical diagnostics lecture on the assessment and management of the non-pregnant patient with abnormal uterine bleeding.

This week we had a special visit from Dr. Darren Braude from the University of New Mexico Department of Emergency Medicine to speak on special topics in airway management. He introduced us to the idea of Rapid Sequence Airway (RSA) and other novel thoughts on extraglottic devices.  We then had a chance to dive in to our regularly scheduled program with topics ranging from tuberculosis to ED operations to complex febrile seizure.  Enjoy!

Morbidity and Mortality Pearls with Dr. Curry

Nephrolithiasis

Incidence in the US is 0.5-1% (lifetime risk 10-15%). There is a 2:1 male predominance and the recurrence rate is fairly high (37% at 1 year, 50% at 10 years and 75% at 20 years).

Patients at risk for poorer outcomes with ureterolithiasis are those with risk factors for diminished renal function, history of difficulty with stones/urologic intervention and symptoms of infection.
