It was another exciting week here in Cincinnati as we kicked off grand rounds with a session on performance improvement during our quarterly leadership curriculum with Dr. McDonough and discussed alternatives to hospital admission and available outpatient resources in the era of high boarding with our Chair, Dr. Pancioli. We finished our day with an excellent discussion on prenatal mental health care with Dr. Wolochatiuk in her R4 Case Follow Up and on uncommon causes of disorientation in Dr. Zalesky’s Taming the SRU lecture.

1. Indications for T pod

  • Blunt trauma + unstable pelvis
  • Blunt trauma + shock + pelvic tenderness to compression
  • Blunt trauma + shock + AMS/inability to evaluate pelvic pain

In patients with blunt trauma who are in shock and have AMS, incidence of pelvic fractures is 10%. In patients who die of blunt trauma during transport, open book pelvis fracture is the #1 cause of death (according to our own QI data)

Here are a few take home points and additions regarding this cherry-colored elixir of wonder: 

  • Think about hydroxocobalamin in your hemodynamically unstable or otherwise SICK patients who have a history of smoke exposure in an enclosed space or a known industrial exposure to CN containing material. This stuff works fast and can be life saving.
