Another great week of Grand Rounds! We started with optimizing our wet reads on MSK xrays then moved into a victorious CPC win in the diagnosis of giant cell artertitis with a patient presenting with CN VI palsy and diplopia. We then discussed the tips and tricks for hernia reduction before hearing a moving story of a harrowing AirCare transport for ECMO cannulation. We moved into R3 small groups covering obstetrics and labor from mother to newborn and finished off with our pediatrics team visiting to discuss emergent cardiac conditions.

We had a jam packed Grand Rounds this week kicked off by Dr. Carleton's airway lecture discussing tools to maximize fiberoptic intubation success. Next Drs. Roche and Plash led us through wilderness medicine small groups discussing plant ingestions and creative extrication techniques. Drs. Murphy-Crews and Bryant participated in a CPC case with a seizing neonate followed by Dr. O'Brien's discussion of blunt neck trauma. Next Dr. Cotton presented data on physician burnout and how to combat it while Dr. Li wrapped things up with an overview of Kawasaki's disease.

This week's Grand Rounds started off with a stress-inducing sim written by the R4s that included a pregnant trauma patient with a positive FAST, an imminent delivery and neonatal resuscitation! Drs. Paulsen and Continenza faced off in a great CPC and Dr. DeVries walked us through the history of US use in ED patients. Dr. Golden rounded out our day with a lecture focused on foot and ankle injuries.

  • According to the AHA, there are no diagnostic EKG changes for NSTEMI
  • ST elevations in II, III and aVF  with depression in V2 represents and inferior-posterior STEMI
  • ST depressions in the precordial leads may represent posterior MI
  • Continuous ST segment trend monitoring may pick up very dynamic ischemic changes (though this is almost never done anymore)
