Welcome to another installment of the Grand Rounds Recap! We had an excellent series of lecturers that we are very excited to share with you. We started off the day with our visiting lecturer Dr. Noble who presented a some excellent pearls on how to better incorporate lung ultrasound in the management of our patients in the ED. Next up Dr. Mand and Dr. Lagasse went head to head in our clincopathologic case series. Dr. Lagasse nailed the diagnosis of Dengue hemorrhagic fever and Dr. Mand then educated us on how it can progress to shock. To follow came Dr. Pulvino who lead us through some small group cases on the utilization of the PERC criteria and Wells score in the evaluation of patient with a potential pullmonary embolism. Next was Dr. Connelly who presented on the evaluation and management of patients with conditions resulting from ascent to high altitudes. To wrap up the day Dr. Lipshaw demystified the pediatric rash patient.

We had an exciting Grand Rounds this week. We started off with our visiting lecturer, Dr. Jennifer Wiler, who gave us an excellent overview of the dollars and cents behind the care that we provide in the emergency department. Afterwards we practice some oral board style cases. Dr. Betz led a challenging triple patient encounter where we had to simultaneously manage a knee dislocation, aortic dissection with involvement of the right coronary artery, and pneumonia in the setting of HIV. Dr. Curry led a riveting case of preeclampsia where identifying the critical history of a recent delivery was critical to initiating the correct management. We wrapped up the day with a simulation led by Drs. Hill, LaFollette, and Lang on in-flight emergencies!

Welcome to another Grand Rounds Recap. This week Dr. Sayal, a visiting lecturer from North York General Hospital in Toronto, Canada, started us off with some pearls and pitfalls for the patient presenting with musculoskeletal complaints. Next up was Dr. Thompson who provided us some tips on how to prevent the most common patient and consultant complaints in the emergency department. Dr. Habib then led us through an interesting case involving an internal degloving injury. We then ended the day with small groups and a simulation led by Drs. We are excited to share our latest installment of the Grand Rounds Recap with you! Dr. Stettler opened the day with a discussion on leadership styles and how they affect our practice in the emergency department. Next up was Dr. Sabedra with a riveting case of new onset heart failure and acute ischemic stroke in a patient in his twenties. Dr. Berger then gave us a shot to the gut, in a good way, with a lecture on colitis. We then delved into the world of pediatrics with a lecture by Dr. Gray who helps us standardize our approach to evaluating for the possibility of child abuse. Dr. Hughes and Dr. Gottula then ended the day with a practice changing summary of the current literature and a new pathway for the management of NSTEMI.

We had an excellent series of lectures this week at our Grand Rounds to welcome in the New Year! The esteemed Drs. Calhoun and Stettler started us off with our Great Debate series pitting Ketofol against Ketamine or Propofol alone for procedural sedation. The winner of this debate? Definitely the audience who got to soak up the expertise from these clinical giants. Next up was Dr. Leenellett who guided us through some community ED cases of early pregnancy vaginal bleeding in our Discharge, Transfer, or Treat lecture series. Dr. Laurence then led us through a small group case based discussion on the indications for massive transfusions in trauma. Then a hush fell over the crowd as Dr. Iparraguierre laid down the CPC gauntlet with a presentation of Myasthenia Gravis for Dr. LaFollete who masterfully identified the Ice Pack Test as the diagnostic test of choice! Next up was Dr. Colmer who gave some tips for the management of severe alcohol withdrawal with his case follow-up lecture. There was no knocking this knowledge train off the tracks as Dr. Urbanowicz led us through a harrowing discussion on Weapons of Mass Destruction. To round out the day Drs. Klaszky, Gauger, and Murphy Crews led us through some small group exercises on the NIH stroke scale, CT angiography of the head and neck interpretation, neurological emergencies jeopardy, and indications for performing a burr hole with some hands on practice (simulation of course!).

We had another excellent Grand Rounds this week! Dr. Carleton started us off with some thrilling airway cases in his continuing Airway Grand Rounds series. Next, we were transported to Africa for a case based discussion on Global Health with Drs Owens, Sabedra, Ventura, and Murphy-Crews. Dr. Skrobut and Chris Shaw then took us through a deep dive of the current literature on the management of upper GI bleeds. Dr. Ham then taught us about ACE-I induced angioedema through the lens of an amazing case of a patient who required a cricothyrotomy to save her life! Next up, we had Dr. Gleimer go up against Dr. Faryar in our Clincal Pathological Case series where we a classic presentation of Addison’s disease in a pediatric patient. Dr. Hunt then led us through small group discussions on the application of the HEART Score in patients presenting to the ED with chest pain. We wrapped up the day with Dr. Isaac Shaw who presented a the management of SVT in a complicated patient.

We had a great Grand Rounds line-up this week - Dr. Whitford started us off with a challenging case of altered mental status that took a variety of twists and turns highlighting the need for reassessment flexibility in your differential diagnosis in the critically ill patient. Next up was Dr. Koehler and Dr. Betz who led us through a case of septic arthritis which presented atypically as altered mental status. Dr. Edmunds then provided us with emergency presentations of pediatric vomiting. Lastly, we closed the day with Dr. Roblee’s excellent case-based discussion on the evaluation and management of hypertension in pregnancy.

Dr. Murphy then discussed hemodialysis access complications followed by Dr. Palmer’s operations update. Dr. Shah led a discussion on the recognition and management of pediatric fractures. This weeks grand rounds started off with our Morbidity and Mortality conference led by Dr. Shaw who led us on an education expedition covering the use of REBOA, surgical vs. medical management in spontaneous miscarriage, ways we can improve our use of the electronic medical record, management of a persistent air leak in a patient with a chest tube, and the role of CT in CAP. Dr. Alwan then provided us an in-depth look at the often untold challenges of life as a refugee as well as the challenges we face in providing care for these patients. This was followed by Dr. Randolph who presented a case of an incredibly complicated patient with a pulmonary embolism to highlight the steps in management of a decompensating patient with a pulmonary embolism before you initiate thrombolysis. Next up was Dr. Gottula and Dr. Hinckley who went head to head with a challenging presentation of acute aortic dissection as part of our CPC lecture series. Dr. Leech then finished up the day with a case based discussion on the utility of the Kocher Criteria in the evaluation of child with an acutely painful hip.

This weeks grand rounds started off with our combined Emergency Medicine and Neurology conference where Dr. Walsh provided us a summary of the current evidence surrounding ICH expansion and reversal of Warfarin and DOACs. This was followed by Dr. Summers who provided suggestions on how we can be more efficient providers in the ED. Dr. Irankunda then presented on the often missed diagnosis of abdominal compartment syndrome. Next up was Dr. Lang who summarized the current evidence for the various available modalities commonly used in treating back pain. Dr. Golden then finished the day with a couple of cases highlighting the challenges in managing the critically ill patient who presents with penetrating neck injuries.

This weeks grand rounds started off with a discussion on the utility of the bougie in airway management by Dr. Carleton. This was followed by Dr. Kiser and Dr. Gensic who led us through a case-based discussion on complex laceration repairs encountered in the community ED setting. Dr. Neth then presented the evidence behind the use of epinephrine and mechanical CPR in out of hospital cardiac arrest. Next up was Dr. Soria who presented a case of Wernicke’s Encephalopathy with some learning pearls about this often under-diagnosed condition. Dr. Goel then masterfully answered the CPC challenge placed by Dr. Li to diagnose a rare case of ocular syphilis. Dr. Berger then finished the day with an overview of necrotizing fasciitis and the LRINEC score.
