Have fun reading this week’s Grand Rounds Recap! Dr. McDonough provided an insightful talk on Physician Depression and Wellness to start our morning. Following this, we had an excellent session on TEG’s multitude of uses in the ED. We had a variety of ophthalmology pearls shared by Drs. Owens, Scanlon, Ventura, and Harty. Then, Dr. Krack, one of Cincinnati Children’s Hospital’s Pediatric EM fellows shared helpful hints in taking care of pediatric patients with Congenital Heart Disease. Next, Dr. Pulvino shared her clinical diagnostics lecture on Platelet Disorders. Dr. McKee finished our day with an exciting talk on medical ethics and how they apply to the Emergency Department. The week started off with our AirCare team discussing indications for procedures performed pre-hospital. The AirCare team continued by taking us through several cases in our quarterly AirCare M&M. We then got some oral boards practice with Drs. McDonough and LaFollette. The day finished with a simulation led by Dr. Hill covering patients who wish to leave AMA as well as the terrifying lengthening QTc.

Annals of B-Pod: An Ocular Emergency

A Case of Retrobulbar Hematoma

The patient is a female in her 60s who presents by EMS after a fall in a parking lot approximately one hour prior to arrival.  She fell forward and landed on her face.  She believes she simply tripped and fell, but she did lose consciousness and does not know how she ended up on the ground.  Per family, she is unsteady on her feet and falls frequently, requiring a cane at baseline.  She reports feeling “weak” but no other symptoms preceding her fall.  She presents with significant right-sided facial trauma and is unable to open her right eye. She has no complaints of blurry vision in her left eye.  She has no headache or other areas of pain or trauma.
