Atypical Headaches

Headaches account for approximately 4 million, nearly 3% of all ED visits annually. [1] We classify headaches as either primary (benign) or secondary, with secondary headaches occurring due to underlying pathology. In the ED, the goal is to alleviate symptoms safely and effectively while excluding dangerous causes of headaches. While nearly 98% of headaches in the ED are primary or benign [2], ruling out secondary causes of headaches is imperative as failing to diagnose correctly may result in significant morbidity or mortality.

This week there were a number of underlying themes between pacemakers, MIs and leadership as well as some other high yield topics sprinkled in. Dr. Broadstock kicked us off with an educational lecture on pacemakers. We combined with our neurology colleagues to discuss how to best take care of basic neuro complaints in the ED in the time of COVID. R3s led an awesome small group on high sensitivity troponin. Dr. Banning shared the lessons she has learned through her own mistakes and failures. A truly heroic resuscitation for a patient in R heart failure by Dr. Skrobut in his Taming the SRU lecture. Lastly, learned about pediatric ophthalmology from Dr. Zamor.

Acute Coronary Syndrome in Pregnancy

Epidemiology

  • Incidence reported at about 6/100,000 deliveries
  • Maternal mortality is between 5-9%
  • 75% are STEMI
  • 2/3rds are anterior wall MI (LAD or LM as the culprit vessel)

Risk Factors

Many of these are typical ACS risk factors but are less prevalent in the pregnant population

  • Older age (>35 years old for pregnancy is considered older age....yikes)
  • Hypertension
  • Diabetes
  • Obesity
  • Smoking
  • Dyslipidemia
